Client Overview
Our client is a board-certified plastic surgeon, specializing in reconstructive surgery and wound care. Beyond his clinical practice, he also works as an expert witness in legal cases, providing testimony on pressure injuries, wound care, and hospital standards of care. His reports and opinions play a key role in defense and plaintiff strategies.
The Challenge
Even for a highly experienced expert like him, reviewing medical records was overwhelming. He faced recurring challenges with his medical record review provider:
- Summaries from his previous service provider were inconsistent and unfocused, making it harder for him to focus on relevant details for the review.
- Conclusions sometimes included statements without any supporting documents and outdated terminology, weakening their defensibility.
- AI-generated medical summaries produced errors and included facts not backed by documentation.
- Key clinical data, such as repositioning schedules, nutrition supplement administration, wound progression, and hydration details were either missing or disorganized.
- Inefficiencies slowed down his ability to prepare timely, credible expert witness reports for litigation.
The client needed a review solution that produces accurate reviews to prepare his reports.
The Solution
We identified that we must combine AI’s efficiency with the clinical judgment of experts to solve these issues. Here’s what we did:
Understanding our client’s needs:
We had a detailed discussion with him to understand how he wanted the medical records summarized, what information would make his review easier, and how he likes the data to be organized for efficient use. Based on this, we created a workflow specifically tailored to his working style and expectations.
Building an AI+human record review framework:
Our AI system handled the hectic tasks of sorting, indexing, and structuring massive records. We provide organized and structured medical summaries/ chronologies to help him interpret medical events. This combination of AI with human-on-the-loop review ensured both speed and credibility in the final summary output.
Implementing the strategies:
Key features of our services included:
- Automated indexing and data extraction- We used AI to go through hundreds of pages of medical records and extract important information, such as hospital admissions, lab results, medications, and wound notes. It ensured all vital facts were organized and presented chronologically.
- Turn & Position Calendars- We created clear turn and position calendars showing when the patient was repositioned and when turns were missed, including the documented reasons. With this calendar, our client could immediately see if proper care was given to the patient, or any gaps in care, allowing the expert to quickly identify the trends. It made his observations about pressure injuries stronger and helped him present his opinions backed by clear evidence.
- Hydration & Nutrition Charts- We tracked fluid intake/output, protein levels, and whether the patient’s diet plan met their needs. Instead of hunting for scattered information, our client could glance at a chart and understand the patient’s hydration and nutritional status over time. This made it easier for him to quickly assess hydration/nutrition details while preparing testimony.
- Wound Color Calendar- Using the notes and photos in the records, we made visual timelines showing how the wounds healed or worsened over time. We extracted wound measurement details from the records and displayed them in a timeline for easy reference and comparison. The client could instantly see the progression, making it easier to explain wound outcomes clearly in his reports and in court.
- Medication & Vasopressor Charts- We mapped medications and vasopressors against patients’ health conditions, like wound healing or sepsis management. In patients with vasopressor administration, pressure ulcer risk can be high. Their position should be changed more often than those who do not get vasopressors. These kinds of details let our client correlate treatments to outcomes in a clear, defensible way, instead of trying to connect the facts from vast documents.
- Nutrition Orders vs. Delivery Audits- We compared the nutrition orders from what was prescribed to what was actually given to the patient. So that the expert could easily compare the prescribed vs. administered nutrition orders.
- Standardized chronologies and conclusions- We organized all findings from the records into clear, chronological summaries and hyperlinked every vital fact to the exact page and provider note. With our well-structured summaries, the expert could prepare court-ready reports.
- HIPAA-compliant infrastructure- Throughout the process, all the records were handled securely with HIPAA-compliant systems. Our stringent data protection measures gave him peace of mind while outsourcing records for the review process.
Execution
- Pilot Phase: Implemented the hybrid workflow on the defense case involving complex pressure injury records.
- Customization: Adjusted templates to fit the expert’s preference for concise, evidence-focused summaries.
- Expansion: Applied the model across multiple cases, including hospital negligence and elder abuse litigation.
The Results
The transformation was significant:
- Report preparation time cut by 60%- With our combined model, our client got flawless and precise summaries. He no longer had to spend long hours sorting through records.
- Improved consistency- We brought consistency to his reports by replacing the disorganized, clumsy summaries he used to receive with clear, structured formats and visual charts that made key details easy to find.
- Higher expert productivity- Our faster turnaround time and precise reports boosted his productivity time. With our support, he could take on more cases confidently without feeling drowned in by mrecord reviews.
- Greater legal defensibility- We ensured that every finding was linked to documented evidence. This helped the client eliminate unsupported arguments and present cases in court with confidence.
- Client satisfaction improved – With our evidence-based summary, he was able to deliver faster, clearer, and more credible reports. This helped him build trust among the attorney clients he worked with.
